When treatments are no longer successful or additional support or pain management is needed, hospice may be the right solution.
Hospice provides compassionate care that focuses on comfort, quality and support for individuals with life-limiting illness, their families and caregivers.
Services are provided in surroundings that are familiar; a private home, assisted living, memory care, skilled nursing care center - wherever is home.
Hospice is covered by Medicare, Medicaid, HMO, private insurance, self-pay and charitable donations. Optage Hospice is Medicare-certified.

Your Circle of Care team will assess needs and provide a personalized plan of care and support that is continually updated. The client, family and caregivers are the center of the care team.
Medical Director - oversees the hospice care plan in collaboration with the hospice team and primary care physician.
Nursing Staff - coordinates the care plan and manages pain and symptoms.
Social Worker - identifies emotional, financial and social support and resources.
Hospice Aid - provides personal care, such as bathing, dressing and other activities of daily living.
Chaplain - supports the spiritual needs of the client, families and caregivers.
Therapists - assist with special needs through physical, speech and other therapies.
Hospice-trained Volunteers - share their gift of time and compassion with the patient and give respite to the family/caregiver.
Bereavement Coordinator - provides grief support to the surviving family, up to 13 months after the loss of a loved one.
